The Missouri 4-H page with all the forms and literature to download. Monroe County should be aware that in a couple of instances we use our forms and not the state ones.
Project Meeting Attendance Form

Monroe County has modified the Project Meeting Attendance Form. The two changes: "Signature of Leader(s)" and changing Case County to Cass County.
Clubs may wish to alter the due date for the form at the top. Forms are required to credit leaders for recognition of their volunteer year with the extension office and are needed by club secretaries to do their books. A member is not credited with completing a project unless this form exists for each project and indicates they completed the project.
